Introduction
Many neurodivergent individuals, especially those with combined Autism and ADHD (AuDHD), are misdiagnosed or overlooked entirely. This is particularly true when symptoms present in ways that mimic more "severe" psychiatric conditions like Dissociative Identity Disorder (DID), schizophrenia, or mania. The truth is: AuDHD—especially when layered with trauma, sensory overload, or chronic stress—can look like something else entirely to the untrained eye. And the consequences of misdiagnosis can be devastating.
When the System Gets It Wrong
Take for example a woman who was diagnosed with DID. Her complex personality traits, shifting moods, emotional intensity, and internal dialogues were interpreted as separate identities. But upon closer inspection, these could also be expressions of masking, trauma-induced role adaptation, emotional dysregulation, and sensory overwhelm—common in women with AuDHD. Her diagnosis may reflect how deeply misunderstood the neurodivergent mind still is.
In my own experience, seeking help resulted in health professionals accusing me of being manic, when I was merely trying to articulate complex ideas and injustices—through an AuDHD lens they did not understand. Their inability to see past their own clinical models meant my AuDHD, and the CPTSD that emerged from years of trauma and misrecognition, were dismissed as psychiatric pathology.
Meena Boppana, an educator and autistic advocate, said: "at the age of 29, I was diagnosed with both autism and ADHD, sometimes referred to as “AuDHD” and noted "The interplay of autism and ADHD led to both diagnoses going under the radar for decades. For the majority of my 20’s, like many other late-diagnosed neurodivergent folks, I was misdiagnosed with bipolar disorder. My lack of coping strategies resulted in frequent meltdowns and emotional distress. However, the bipolar diagnosis never seemed to fit, as my mood could change dramatically in a matter of hours, not weeks. Ultimately, I was only formally diagnosed with AuDHD following a self-diagnosis based on content on my TikTok newsfeed.".
The Schizophrenia Connection
There are many documented cases where highly sensitive or gifted neurodivergent people were misdiagnosed with schizophrenia. My uncle, for example, was described as gifted but emotionally intense, and he ended up institutionalized with a diagnosis of schizophrenia. Today, in hindsight, his behaviors resemble what we now recognize as autistic traits mixed with ADHD-like bursts of energy, obsession, and confusion under stress.
Another case I saw on television told the story of a man who spent years with an incorrect diagnosis of paranoid schizophrenia—only to later be reevaluated and found to be AuDHD. Why It Happens
- Many clinicians are not trained in how neurodivergence, especially AuDHD, presents in adults
- Women are especially prone to misdiagnosis due to masking and internalizing symptoms
- Trauma (including from childhood or systemic neglect) can compound and obscure the neurodivergent traits
- People with AuDHD often show intense emotional expression, rapid speech, tangential thinking, or dissociation—traits that may be pathologized as mania, BPD, or DID
Connected Conditions: The Neuroimmune Overlap
Emerging evidence shows a high overlap between neurodivergence and conditions such as:
- Fibromyalgia
- Chronic fatigue syndrome (CFS/ME)
- Allergies and asthma
- Autoimmune dysregulation
These conditions are often co-expressed in sensitive nervous systems—what some researchers are beginning to call “neurocomplexity.”
Reclaiming the Narrative
It's time we stop assuming that complex minds are broken ones. Misdiagnosis doesn't just lead to wrong treatments—it leads to disempowerment, stigma, and sometimes lifelong institutional trauma. The stories of various AuDHD people point to a systemic failure: a medical model that doesn't yet have a framework for the full spectrum of neurodivergence.
Conclusion
AuDHD is real. It’s underdiagnosed. And it’s too often misunderstood. It is time for the mental health system to catch up—to replace misdiagnosis with deep listening, curiosity, and frameworks informed by trauma and neurodiversity. Only then can we prevent brilliant, sensitive, divergent minds from being lost to misunderstanding.
